Requesting Health Care Records:
Consumers may request copies of their health care records maintained by Forensic & Clinical Psychology Center by submitting a written request by mail.

Requests should include the consumer’s full name, date of birth, dates of service (if known), and a signed authorization if records are to be released to a third party. Requests are processed in accordance with applicable Texas and federal law.
